Community-Based Support
This service helps individuals, with or without a caregiver, at home or outside, to enhance independence, productivity, family functioning, and community inclusion as per their plan. They include one-on-one assistance with community activities, training, and supervision for essential self-care, social skills, and daily tasks.
Community Inclusion
Activities provided outside a participant’s home to support their involvement in educational, enrichment, or recreational activities outlined in their Service Plan, aiming to promote community inclusion. These services are conducted in a group setting with a maximum of six individuals.

Day Habilitation
This service offers education and training to develop the skills necessary for community participation, in line with the participant’s Service Plan. This includes support for problem-solving, self-help, social, adaptive, daily living, and leisure skills in environments designed to foster skill acquisition, positive social behavior, and independence. These services are provided during daytime hours and exclude employment-related training. Day Habilitation may be offered in center-base

Individual Support
This service provides individual support to help acquire and improve behavioral, self-help, and adaptive skills essential for successful home and community functioning, using evidence-based and data-driven approaches. Support is delivered at home, a relative's home, or other community settings, focusing on behavioral support, skill development, daily living activities, and community inclusion. The goal is to promote goal attainment, adaptive functioning, and communication skill
Transportation
Our Community Transition Services aid individuals in moving from institutional or group living to a less restrictive or private residence. We cover one-time set-up expenses for a smooth transition to a living arrangement where they manage their living expenses. This service enables access to specified activities and resources outlined in the service plan. Whenever feasible, we engage family, neighbors, friends, or community agencies that provide this service at no cost.
